How do I get a Utah Medical Cannabis Card?
Make an appointment with us.
Fill out the State of Utah’s medical cannabis card form by clicking here.
Either be seen in person (if you are a new patient or if you are renewing your card with a new provider than last time) or be seen via telehealth appointment through our website (cannabis card renewals).
What are the requirements to get a medical cannabis card in Utah:
Must be a Utah resident over 21 years of age.
Must have at least one qualifying condition.
Submit an application online (click here).
Meet in-person with a qualified medical provider (QMP) who is registered with the Utah Department of Health and Human Services to recommend medical cannabis. Telehealth appointments are available for those on hospice, residential facility, physical rehab, nursing home or for the yearly renewal of your cannabis card.
At your online or in-person visit the qualified medical provider will certify your eligibility online.
You will pay the State of Utah required $15 fee online.
What conditions are eligible for a Utah Medical Cannabis Card?
There are 16 conditions Utah recognizes to be eligible for a Utah Medical Cannabis Card:
1. Persistent Pain (>2 weeks not adequately managed despite treatment with conventional medications or treatments (other than opioids)).
2. Post Traumatic Stress Disorder (PTSD) - must have documentation that it has been diagnosed by a health care provider
3. Acute pain that is expected to last for 2 weeks or longer
4. Epilepsy or debilitating seizures
5. Crohn’s Disease or Ulcerative Colitis
6. Persistent Nausea (not significantly responsive to treatment)
7. Multiple Sclerosis or persistent and debilitating muscle spasms
8. Cancer
9. Cachexia (major weight loss)
10. Amyotrophic Lateral Sclerosis (ALS)
11. Alzheimer’s Disease
12. HIV or AIDS
13. Autism
14. A terminal illness when a patient is expected to live <6 months
15. a condition requiring hospice
16. Rare condition

How long is a medical cannabis card good for?
One year from the date it is issued. The Qualified Medical Provider (QMP) can require more frequent follow ups (6 months or 3 months) if deemed medically necessary
Where can medical cannabis be purchased in Utah once I have a card?
As of Jan 2025 there are 15 dispensaries, five of which provide home delivery.
You are not allowed to have medical cannabis from another state per Utah law.

How long does it take to get the card?
After you’ve filled out your cannabis application and seen our providers it will take less than 3 business days or sooner. Please note we will not submit your final application until after you have paid for your visit.
Is medical cannabis legal under federal law
No. But it is legal to use in the state of Utah only if you get a Medical Cannabis Card from a Qualified Medical Provider (QMP)
What is a Qualified Medical Provider
Utah-licensed Physician, nurse pracititoner (NP) or Physicians Assistant who are licensed to prescribe a controlled substance. They attend further training for this licensing.
